The initial visit will be a thorough evaluation. We will discuss your symptoms, previous and current level of function, past medical/injury history and other factors that may be contributing to the current issue and your goals. I will perform a comprehensive examination, explain my findings and your plan of care. Following the visit I will send written information of the Home Exercise Program, educational materials and any modification to your activity level that I recommend.
All follow up appointments are also an hour. I encourage communication between visits so that you don’t have to wait until your next session to get any questions answered.
Physical Therapy has direct access, meaning the state doesn’t require a prescription for treatment. This can save both time and money. However, you may want to check with your insurance to see if they require a prescription to get reimbursement.
CustomCare PT is an “Out of Network Provider”, which means that I do not have contracts with insurance companies. You will pay for the visit up front then I will provide you a Superbill that can be submitted to your insurance for reimbursement. Contact your insurance company to understand the process and to determine the amount they will reimburse. Any reimbursement will be sent directly to you.
Being out of network allows us to focus exclusively on your goals without having an insurance company dictate what treatments can be offered or how many visits will be allowed.
